Job Description

St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. 

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. 

Position Summary

St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager to join our Project Protect team. In this vital role, you'll assist with immediate housing needs, connect clients to community resources, and empower them with life skills for greater stability. Your expertise in crisis intervention and financial guidance will be essential in fostering overall. The case manager will assist with housing placements, conduct monthly in-home visits, and develop individualized treatment plans to empower clients toward self-sufficiency. You'll link clients with essential community services, help them navigate daily living skills, and provide transportation to appointments. A collaborative spirit is key as you work with community partners, attend meetings, and tackle departmental goals. The core working hours are Monday through Friday, 8:00 am to 4:30 pm.

Overview and Responsibilities

• Outreach and engage with newly referred program clients

• Assist with initial housing placement and relocation as needed

• Conduct monthly in-home visits to observe indicators of client progress

• Write and implement collaborative individualized treatment plans with client

• Complete collaborative monthly budget and quarterly crisis plan with client

• Link client with supportive community services, enabling as much self-sufficiency as possible

• Problem solving/assistance with community integration skills (shopping, managing money, public transportation, etc.)

• Use personal vehicle to transport clients to and from appointments and to access community resources

• Provide community support to clients with varying levels of mental health, physical health, and substance use concerns

• Act as a liaison and consult with community agencies, landlords, and clients to maintain stability

• Network and build relationships with community partners

• Implement early identification and research-based intervention practices

• Complete departmental goals as outlined by grants

• Collaborate with other members of the Housing program

• Make referrals to other SPC programs as appropriate

• Update internal database daily to document services provided and client progress

• Create and maintain an organized hard copy file for each client

• Conduct quarterly internal file reviews

• Assist with agency-wide and seasonal projects as needed

• Responsible for agency policy regarding work hours, building security, and all written personnel standards

• Attend and participate in all appropriate St. Patrick Center meetings

• Networks with federal, state, and local agencies

• Other duties as assigned.

 

Specific Responsibilities for Project Protect case manager:

• Connect to parenting classes and family counseling as needed

• Assist families in crisis by utilizing crisis intervention techniques

• Ensure that all school-aged children are enrolled in school, assist with enrollment

• Work directly with Children’s Division and other parties involved in reunification process

Knowledge and Experience Requirements

• Bachelor’s degree in social work or related Human Services field or 4 to 6 years equivalent professional experience required.

• Requires driver’s license (MO residents require Class E)

• Requires reliable vehicle and valid and current auto insurance

• Experience in case management

• Experience with trauma-informed services

• Knowledge and understanding of community agencies

• Knowledge of federal, state, and agency policies of confidentiality
