Schedule: As Needed for Coverage / PRN Shift: Shifts when scheduled will be during the week Monday - Friday 8:00am - 4:30pm, no weekends, no holidays Location: 500 Remington Boulevard, Bolingbrook, IL 60440 The community you'll be caring for: UChicago Medicine AdventHealth Bolingbrook The Role You'll Contribute Responsible for providing integrated clinical and distributive pharmacy services, decentralized inter-professional pharmacy practice, and direct patient care programs. This position leads general and specialty direct patient care programs, advanced drug management services, and medication use systems throughout the institution. This role completes medication order scrutiny, drug profile review, patient monitoring, and participates in medication safety systems, regulatory compliance initiatives, patient quality services, and disease outcomes programs. Other responsibilities include delivering medication therapy management consult services and disease state management services as needed.
The Value You'll Bring To The Team - Performs all pharmacist functions, actively participating in established standard work parameters and providing coverage of all pharmacy areas.
- Oversees and ensures appropriate utilization of medication automation, technology, and computer systems.
- Accurately manages medication inventories, and prepares and dispenses patient-specific medication doses.
- Supervises technician and intern practice, prioritizes and organizes workflow, and checks accuracy of pharmacy technician activities.
- Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems.
- Verifies and enters medication orders, proactively communicating with medical staff to resolve problems.
- Provides accurate, safe, and timely medication therapy, and promotes cost-effective drug therapy.
- Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults.
- Identifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors.
- Leads and facilitates pharmacy practice programs, direct patient care services, and medication systems.
- Other duties as assigned.
Qualifications The expertise and experience you'll need to succeed : - Bachelor's in Pharmacy Minimum Required
- Five or more years of Work Experience Required
- Strong verbal and written communication, organizational, problem-solving, and customer service skills
- Flexibility and willingness to adapt to changes as necessary
- Ability to work well without supervision and to supervise personnel
- Ability to interact constructively with supervisors, peers, and support personnel
- Ability to analyze data and follow up and resolve discrepancies
- Ability to keep accurate and complete records
- Ability to teach, precept, and facilitate experiential training
- Initiative to keep abreast of developments in the provision of pharmacy services
- Initiative to keep abreast of new information/products/developments in the field of pharmacy
- Proficiency in general hospital pharmacy operations and regulatory compliance
- Proficiency with hospital computer systems, pharmacy technology, and pharmacy automation
- Proficiency in hospital pharmacy practice, patient-centered care, advanced pharmacy practice, medication therapy management, and evidence-based medicine
- Working knowledge of the American Society of Health-System Pharmacists Accreditation Standards for Pharmacy Residency Programs
- Ability to coordinate and lead small groups of people in determining policy, implementing procedures and demonstrating progress
- Leadership qualities essential to operate effectively within the institution and advance the practice of pharmacy
- Must be able to read, write, speak, and have good comprehension of conversational English
- Basic computer skills and knowledge of Microsoft Office
- Ability to define operational or program problems/needs and solutions
- Ability to integrate clinical and distributive pharmacy services with quality improvement, teaching, research, and professional development activities
- Expertise using Cerner pharmacy solutions and services
- Expertise using Pyxis automated dispensing technology
- Working analytical and data management skills
- Working knowledge of Microsoft Office Outlook, Word, Excel, and PowerPoint
- Working knowledge of the accreditation requirements of Det Norske Veritas and International Organization for Standardization quality management systems
- Working knowledge of the American Society of Health-System Pharmacists National Practice Standards
- Licensed Pharmacist (RPH) Required
